INVITATION FOR EXPRESSION OF INTEREST AND TENDER

The University of Port Harcourt Teaching Hospital Port Harcourt invites interested, competent and reputable contractors to apply for expression of interest and tender for the under listed projects

(i) Structural Repairs of Leakages

(ii) Re-painting of the Hospital Building

(iii) Purchase of critical Hospital Equipment for Clinical Departments REQUIREMENTS:

Application for Pre-qualification should be accompanied with the following:

(i) Evidence of registration of company with Corporate Affairs Commission.

(ii) Company Tax clearance for the last three years

(iii) Value added Tax Registration Certificate

(iv) Evidence of Financial capacity to execute this project e.g. Audited Accounts for the last three years and reference letter from the company’s bankers

(v) List of similar verifiable and successfully executed jobs by the company with the location, letter of award and job completion certificates within the last three years.

(vi) List of Managerial, Technical and Administrative staff of the company including their resume and copies of relevant professional qualifications where necessary.

(vii) Evidence of compliance with the pension Act 2004

(viii) Evidence of compliance with the Industrial Training Fund ITF Act 2011.

(ix) Sworn affidavit that none of the Directors have been convicted in any court of law for criminal offence, fraud and financial impropriety.

(x) Evidence of payment of non-refundable tender fee of N50,000.00 (Fifty Thousand Naira) only payable to the University of Port-Harcourt Teaching Hospital’s Treasury Single Account (TSA) at the Central Bank of Nigeria Account Number 012050431018 original copy of the Hospital receipt should be attached to the Tender document.

SUBMISSION OF EXPRESSION OF INTEREST (EQI) AND TENDER:

The expression of interest and tender documents must be in separate wax-sealed envelopes, each appropriately addressed.

(b) Expression of Interest for:………………………………… And both contained in a larger envelope. The bigger envelope should be marked expression of interest and tender for…………..boldly written on the top right hand corner of the envelope.

The tender documents should be dropped in the tender box in the office of the Head of Procurement Unit, Department of Works Services, University of Port Harcourt Teaching Hospital Port Harcourt not later than 12noon on 1st August, 2017.
